What is usually going on

The spine is a stack of joints, and each one is meant to move a small amount in several directions. When one or two of them stop moving properly — after a long stretch at a desk, a poor night's sleep, a lift done in a hurry, or years of a habitual posture — the surrounding muscles tighten to protect the area. That protective tension is often what you actually feel: a band across the low back, a knot between the shoulder blades, a neck that will not turn one way.

Because the muscle guarding is a response rather than the cause, treating only the muscle tends to give short-lived relief. Massage, heat and stretching feel good and often help, but if the joint underneath has not started moving again the pattern returns within days. This is the most common reason people tell us they have been managing the same ache for years.

What the first visit looks like

Nothing is adjusted before we understand what we are dealing with. The visit starts with an unrushed conversation: when it started, what makes it worse, what you have already tried, what your work and sleep look like, and what you want to get back to doing.

Then we examine. Posture analysis, range-of-motion testing in each direction, orthopaedic and neurological tests where they are relevant, and joint-by-joint palpation to find which segments have stopped moving. We take X-rays on site when the examination warrants it — for example when there is a history of trauma, or when findings do not fit a straightforward mechanical pattern — and not as a default for everyone.

How we treat it

The adjustment restores motion to the segments that have stopped moving. We use Gonstead, Diversified, Thompson Drop and Activator instrument methods, and the technique is matched to your body and comfort rather than to habit. Patients who dislike the sensation of a manual adjustment are treated with low-force instrument methods that achieve the same aim more gradually.

Around the adjustment we treat the soft tissue that has been compensating, using hands-on work, muscle stimulation and PEMF where it helps the area settle faster. All of these are listed under services.

The part that determines whether relief lasts is what happens between visits. You will leave with two or three specific things to change — a loading exercise, a stretch that targets the restricted direction, and usually one ergonomic adjustment to a desk, car seat or pillow. We keep the list short deliberately, because a short list gets done.

How long it takes

Acute mechanical back and neck pain often eases noticeably within the first few visits. Longer-standing patterns take longer, because the joint restriction and the muscle habit have to change together. What we can promise is a plan with a defined end point rather than an open-ended schedule.

We re-examine as we go and measure the same things we measured on day one, so progress is a comparison rather than an impression. When you have met the goals you came in with, care stops. Some patients choose to come in periodically after that; plenty do not, and that is a reasonable choice.

Follow-up visits usually take ten to fifteen minutes. Adjusting hours are Monday and Wednesday 8—10am and 3—6pm, Tuesday and Thursday 3—6pm, and Friday 8—11am.

When it is not a chiropractic problem

Some back and neck pain needs a different kind of care, and part of the examination is ruling that in or out. Pain that wakes you every night regardless of position, unexplained weight loss, fever, progressive weakness or numbness, loss of bladder or bowel control, or pain following significant trauma all warrant medical assessment first.

If your presentation points somewhere other than our office, we will tell you at the first visit and help you get to the right person. Declining to treat is part of the job. Parents asking about children, and patients who are pregnant, should read pregnancy and pediatric care, where the approach differs considerably.

The habits that keep it coming back

Almost everyone we treat for recurring back or neck pain has one or two daily positions doing most of the damage. A monitor set below eye level pulls the head forward for eight hours a day. A car seat reclined a little too far rotates the pelvis backward and flattens the low back for a long commute. A soft mattress or a pillow of the wrong height holds the neck in end-range rotation all night.

None of these cause pain on their own. They matter because they load the same tissue in the same direction for hours at a time, and the spine adapts to what it does most. Changing one of them is usually worth more than any stretch we could give you.

We go through your day at the first visit and pick the one or two positions worth changing, rather than handing over a generic ergonomic sheet. Do I need a referral?

No. California is a direct-access state for chiropractic, so you can book without a referral from a medical doctor.
