Filling them in

Each form starts with a consent tick box, and no questions appear until it is ticked. If a form looks empty, that is why. They do not save part-way, so set aside the time rather than starting and coming back.

Signatures

There are signature boxes to sign with a finger or a mouse, and a printed name and email beside each. All of them have to be completed or the form will not submit.
